Acetaminophen, Caffeine, Dihydrocodeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Devil's Claw interacts with.

Theoretically, devil's claw might increase levels of drugs metabolized by CYP3A4.
In vitro research shows that devil's claw might inhibit CYP3A4, although this has not been reported in humans.

Here's the gist with this combo pill (acetaminophen, caffeine, and dihydrocodeine). Lab studies hint that devil's claw might slow down a liver enzyme called CYP3A4, which plays a part in clearing dihydrocodeine. If that enzyme slows, a little more of the opioid could hang around, which in theory means more drowsiness, dizziness, constipation, or slowed breathing. Worth noting, the caffeine in this product is also handled partly by your liver, so levels there could nudge up too.

I want to be honest about the strength here: this is only seen in test-tube research, never confirmed in real people, so it may not amount to much. Still, since this medicine contains an opioid, it's a smart idea to run it past your pharmacist or doctor before adding devil's claw.
