Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the piece of this combo that matters most: the caffeine portion is cleared mainly by a liver enzyme called CYP1A2. In lab studies (test tube only, not in real people), chanca piedra seemed to speed that enzyme up. If that were to happen in your body, caffeine could be broken down faster, meaning you might notice a bit less of its usual pick-me-up effect. That's the only well-supported direction here, and honestly the evidence is thin.

I want to be straight with you: this hasn't been seen in humans, so it's mostly theoretical. The acetaminophen and dihydrocodeine parts aren't really driven by this enzyme, so I wouldn't expect changes in pain relief or in side effects like drowsiness or constipation. Still, it's worth a quick chat with your pharmacist before combining them.

Here's the thing to keep in mind with this combination pill. Lab studies suggest chanca piedra may block an enzyme called CYP3A4, which your body uses to help clear several medicines. The caffeine and, to some degree, the dihydrocodeine portion lean on this enzyme. If it's slowed down, those pieces could build up a little, meaning you might feel more jittery or wired from the caffeine, or more drowsiness, dizziness, constipation, or nausea from the opioid side.

I want to be honest though: this is only seen in test-tube research, never reported in real people, so it's more theoretical than proven. Still, since dihydrocodeine is an opioid, it's worth a quick chat with your pharmacist or doctor before pairing them.
