CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

Propranolol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP1A2 to help break down propranolol. In lab studies, chanca piedra seems to speed that enzyme up. In theory, that could clear propranolol out of your system a bit faster, leaving less of it to do its job. Propranolol is the active drug itself (not a prodrug that needs activating), so faster breakdown points toward a weaker effect.
What might that look like? Blood pressure or heart rate creeping back up, more frequent migraines, or chest pain or tremor not being as well controlled. I'll be honest though, this is only seen in test-tube research and has never been reported in real people, so the risk is uncertain. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before combining them.

ANTIHYPERTENSIVE DRUGS

Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the simple version: chanca piedra might nudge your blood pressure a little lower, and propranolol already lowers blood pressure and slows your heart rate. So in theory, taking them together could add up to a slightly bigger drop than you'd expect on the medication alone. If that happened, you might feel lightheaded, dizzy, unusually tired, or notice a slower pulse.
That said, I want to keep this in perspective. This concern comes mostly from animal studies, and in a small human study of people with high blood pressure, chanca piedra didn't actually lower their pressure over 10 days. So the real-world risk looks low. Still, it's worth mentioning to your pharmacist or doctor, and checking your blood pressure at home if you decide to use both.

This intravenous (IV) form of propranolol is used in hospital or clinical settings to treat dangerous heart rhythm problems. It is generally reserved for life-threatening arrhythmias (abnormal heart rhythms) or those that occur during anesthesia.
Specific conditions it treats include fast heart rates coming from the upper chambers of the heart (supraventricular tachycardia), certain arrhythmias caused by digitalis toxicity, dangerous ventricular rhythms that haven't responded to other treatments, and fast heart rates caused by excess adrenaline-like substances during surgery.
