CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Diltiazem falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: your body relies on a liver enzyme called CYP3A4 to break down diltiazem and keep its level steady. In lab testing, chanca piedra appears to slow that same enzyme down. If that happened in real life, diltiazem could clear out more slowly and build up a bit, which might mean more of its usual effects, things like low blood pressure, dizziness, a slower heart rate, flushing, or swollen ankles.
I want to be honest about how weak this evidence is. So far it's only been seen in test tubes, not in people, so it's theoretical. Still, since blood pressure and heart rhythm matter, I'd run it by your pharmacist or doctor before adding chanca piedra rather than guessing.

ANTIHYPERTENSIVE DRUGS

Animal research suggests that chanca piedra can decrease blood pressure. However, this effect was not observed in most hypertensive patients treated with chanca piedra for 10 days.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: chanca piedra may gently lower blood pressure on its own, and since diltiazem (Cardizem) is already working to relax your blood vessels and ease your heart's workload, the two could in theory add up. If your pressure dipped too low, you might feel lightheaded, dizzy, tired, or notice your heart pace feeling off when you stand up quickly.
That said, the evidence here is weak. This is mostly a theoretical concern based on animal studies, and a small human study didn't see much blood pressure drop after 10 days. So for most people this is unlikely to cause trouble. Still, if you'd like to try chanca piedra, check with your pharmacist or doctor first, and keep an eye on your readings at home so any changes get caught early.

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL
Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).
Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
