Ivabradine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, use of chanca piedra might increase the levels and clinical effects of CYP3A4 substrates.
In vitro research shows that chanca piedra extract inhibits CYP3A4. Theoretically, chanca piedra might increase the levels of CYP3A4 substrates. This interaction has not been reported in humans.

Here's the simple version. Your body clears ivabradine mainly through a liver enzyme called CYP3A4. In lab studies, chanca piedra appears to slow that enzyme down. If that happened in a real person, ivabradine could build up higher than expected, which may push its heart-rate-lowering effect too far.

With ivabradine, that could mean an unusually slow heartbeat, dizziness, tiredness, or those visual brightness flashes (called phosphenes) it's known for. That said, this concern comes only from test-tube research and has not been reported in actual patients, so I wouldn't panic. Before combining them, just run it by your pharmacist or doctor so they can keep an eye on your heart rate and how you feel.

About Ivabradine

What Is Ivabradine Used For?

Brand names: Procoralan, Coralan, Corlentor, Corlanor

Ivabradine is used for stable, symptomatic chronic heart failure in adults to reduce the risk of being hospitalized for worsening heart failure. It is for adults with a reduced ejection fraction (the heart's pumping strength is low, 35% or less), who are in normal sinus rhythm with a resting heart rate of 70 beats per minute or higher. They must also be on the highest beta-blocker dose they can tolerate, or be unable to take beta-blockers.

Corlanor (the brand) is also approved to treat stable, symptomatic heart failure due to dilated cardiomyopathy (a weakened, enlarged heart muscle) in children 6 months and older who are in sinus rhythm with an elevated heart rate. The generic ivabradine tablet labels list the adult use only.
