Bivalirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the thing to keep in mind with this pairing. Bivalirudin is a powerful blood thinner given through an IV, usually in the hospital during heart procedures like angioplasty. Its whole job is to prevent clots, so anything that adds to that effect could nudge up your bleeding risk.

Chanca piedra contains a plant compound that, in test-tube studies only, slowed platelets from clumping together. This has not been shown in actual people, so the concern is purely theoretical. Still, on top of a strong anticoagulant like bivalirudin, the worry would be easier bruising or bleeding (from the IV site, gums, or nose). Since bivalirudin is given in a closely monitored hospital setting, mention any supplements you take to your care team so they have the full picture.
