Dipyridamole falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might increase the risk of bleeding when used concomitantly with anticoagulant/antiplatelet drugs.
In vitro research suggests that methyl brevifolincarboxylate, a constituent isolated from chanca piedra, can inhibit platelet aggregation. This effect has not been reported in humans.

Here's the thing to keep in mind with this pairing: dipyridamole already works by making your platelets less sticky, which is exactly how it helps prevent clots after a heart valve replacement. Chanca piedra contains a plant compound that, in test tube studies, also slowed platelets down a bit. Stack those two together and the theory is you could see a little extra bleeding tendency.

I want to be honest about how soft this evidence is. The anti-platelet effect of chanca piedra has only been seen in a lab dish, never shown in actual people, so this is a precautionary flag, not a proven problem. Still, watch for things like easy bruising, nosebleeds, bleeding gums, or cuts that take longer to stop. If you notice any of that, or before you start the herb, please check in with your pharmacist or doctor.

Dipyridamole tablets are used alongside warfarin (a blood thinner) to help prevent dangerous blood clots from forming after cardiac valve replacement surgery. It is not used alone for this purpose — it works as an add-on to anticoagulant therapy.

Dipyridamole Injection is used as an alternative to exercise during thallium myocardial perfusion imaging — a type of heart scan that checks blood flow through the coronary arteries. It is given to patients who are unable to exercise adequately for the test.
