Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Chanca Piedra interacts with.

Theoretically, chanca piedra might reduce the levels and clinical effects of CYP1A2 substrates.
In vitro research shows that chanca piedra extract increases CYP1A2 activity. Theoretically, chanca piedra might increase metabolism of CYP1A2 substrates and lower serum concentrations. This interaction has not been reported in humans.

Here's the gist for nortriptyline: one of the liver enzymes that helps clear it from your body is called CYP1A2. In lab dish studies, chanca piedra seemed to speed up that enzyme. If that held true in real people, your body might break nortriptyline down a little faster, which could nudge its blood levels down and make it work less well for your mood.

I want to be straight with you, though. This is only seen in test tube and animal research, not in actual patients, so it's more of a theoretical concern than a proven problem. Nortriptyline isn't a prodrug, so no flip in direction here. Before adding chanca piedra, just check with your pharmacist or doctor so they can keep an eye on how you're feeling.
