Piracetam and Bivalirudin

Here is the interaction we found between these two, with the evidence behind it.

Major

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Bivalirudin

Bivalirudin falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Piracetam interacts with.

Theoretically, concomitant use of piracetam and anticoagulant/antiplatelet drugs might increase the risk of bleeding.
Preliminary clinical research shows that a single dose of piracetam inhibits platelet aggregation.
Severity Major Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: bivalirudin (Angiomax) is a strong blood thinner given through an IV during procedures like angioplasty, so it's already working hard to prevent clots. Piracetam appears to make platelets a little 'stickier-resistant,' meaning it can slow how your platelets clump together. Stack that on top of bivalirudin and you may tip further toward bleeding.

In real terms, that could mean more bruising, bleeding at the catheter or IV site, nosebleeds, or harder-to-stop bleeds. The good news is this concern rests on early research and is mostly theoretical, so it's a flag for caution rather than panic. Since bivalirudin is typically used in a closely monitored hospital setting, your care team is already watching for bleeding. Always tell them about any piracetam use beforehand.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Bivalirudin page

“Bivalirudin (Angiomax) is a strong intravenous blood thinner used during heart procedures like angioplasty, and because bleeding is already its main built-in danger, this list deserves genuine care. All 202 supplements on it interact the same basic way: they thin the blood or blunt platelets themselves, stacking bleeding risk on top of…”

About Piracetam

What is Piracetam?

  • Memory and cognition ('nootropic')
  • Cognitive decline and dementia
  • Myoclonus
  • Dyslexia in children
  • Vertigo

Piracetam is a synthetic 'nootropic' drug studied mainly for cognitive decline, myoclonus, and certain neurological conditions, but evidence for boosting memory in healthy people is weak. It is a prescription medicine in many countries and is not an approved drug or dietary supplement in the United States, so quality and safety of products sold online vary. Talk to a doctor or pharmacist before using it.
