Why this applies to Indinavir

Indinavir falls under Cytochrome P450 3A4 (Cyp3A4) Substrates — the group Puravive interacts with.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Here's the situation with Puravive and your indinavir (Crixivan). Puravive contains Panax ginseng, and indinavir is cleared from your body by a liver enzyme called CYP3A4. Ginseng has shown mixed effects on this enzyme, sometimes seeming to slow it down and other times speeding it up, so honestly we can't predict which way it would push your indinavir levels.

That uncertainty is the concern. If your indinavir levels drift up, you could see more side effects like kidney stones or stomach upset. If they drift down, the medicine may not control HIV as well, which risks resistance. The evidence here is weak and the listed risk is minor, but because keeping HIV suppressed matters so much, I'd check with your doctor or pharmacist before adding Puravive.

P-Glycoprotein Substrates

Indinavir also falls under P-Glycoprotein Substrates.

Puravive contains quercetin. Theoretically, concurrent use of quercetin and P-glycoprotein substrates might alter the effects and adverse effects of the drugs. There is preliminary evidence suggesting that quercetin inhibits the P-glycoprotein efflux pump in the gastrointestinal tract. This inhibition could potentially increase the bioavailability and serum levels of drugs transported by this pump. For example, a small study in healthy volunteers found that pretreatment with quercetin increased bioavailability and plasma levels following a single dose of cyclosporine (Neoral, Sandimmune) (source: https://pubmed.ncbi.nlm.nih.gov/15581264/). However, other small studies have shown that quercetin might decrease the absorption of talinolol, another substrate of the gastrointestinal P-glycoprotein efflux pump. Interestingly, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase) in another small study (source: https://pubmed.ncbi.nlm.nih.gov/16945047/). The reason for these discrepancies is not entirely clear. Until more is known, it is advisable to use quercetin cautiously in combination with P-glycoprotein substrates.

Here's the gist: Puravive contains quercetin, a plant compound that may block a gut transporter called P-glycoprotein. Indinavir relies on that same pump as it moves through your intestines, so in theory, blocking it could let a bit more of your dose into your bloodstream. Indinavir isn't a prodrug, so higher levels would simply mean stronger drug effects, and possibly more side effects like kidney stones, belly upset, or that yellowish tint to the skin or eyes.

I want to be straight with you, though. This is theoretical and based on weak, mixed lab and small-study evidence. One quercetin study barely changed a similar HIV drug at all. Since indinavir is so important to keep working steadily, please loop in your doctor or pharmacist before adding Puravive.

  • Indinavir is used along with other medications to treat human immunodeficiency virus (HIV) infection. Indinavir is in a class of medications called protease inhibitors. It works by decreasing the amount of HIV in the blood. Although indinavir does not cure HIV, it may decrease your chance of developing acquired immunodeficiency syndrome (AIDS) and HIV-related illnesses such as serious infections or cancer. Taking these medications along with practicing safer sex and making other lifestyle changes may decrease the risk of transmitting the HIV virus to other people.
