Alogliptin falls under ANTIDIABETES DRUGS — the group Colocynth interacts with.

Theoretically, colocynth might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that colocynth fruit powder reduces fasting blood glucose levels in patients with type 2 diabetes who are also taking antidiabetes drugs.

Here's the short version: colocynth seems to lower blood sugar on its own. Small studies found that people with type 2 diabetes had lower fasting glucose when they added colocynth fruit powder to their diabetes medicine. Since Alogliptin (Nesina) already works to bring blood sugar down, stacking the two could push your sugar too low. That's called hypoglycemia, and it can leave you feeling shaky, sweaty, dizzy, confused, or unusually tired.

I'll be honest with you, the evidence here is fairly thin and partly theoretical. But there's a bigger concern: colocynth itself is genuinely toxic, even in small amounts, and most pharmacists (myself included) would steer you away from it entirely. Please don't start or stop anything on your own. Talk with your doctor or pharmacist first.

Alogliptin is not used for type 1 diabetes or diabetic ketoacidosis, as it would not be effective in those situations.
