Here's the concern with these two together. Benzthiazide (Exna) is a thiazide-type water pill, and one of its known effects is lowering your potassium level. Colocynth works as a harsh laxative, and strong laxatives can flush potassium out of your body too. So you'd potentially be losing potassium from two directions at once.

If potassium drops too low, you might feel muscle weakness or cramps, fatigue, or notice an irregular or skipped heartbeat. I'll be honest with you though: this warning is theoretical, based on how these things work, not on solid human studies. The bigger issue is that colocynth is genuinely toxic and most pharmacists, myself included, would steer you away from it entirely. Please talk with your doctor or pharmacist before using any product containing it.
