Theoretically, colocynth might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Small clinical studies show that colocynth fruit powder reduces fasting blood glucose levels in patients with type 2 diabetes who are also taking antidiabetes drugs.

Here's the gist for glucose, the sugar given (often by IV or by mouth) to raise low blood sugar or to treat hypoglycemia. Colocynth seems to push blood sugar in the opposite direction. Small studies suggest its fruit powder can lower fasting blood sugar in people with type 2 diabetes. So taking colocynth alongside something meant to boost your sugar could partly work against it, potentially leaving your sugar lower than expected.

This is fairly low-quality evidence, so I wouldn't panic. The bigger worry is that colocynth itself is genuinely toxic and a harsh laxative, and most pharmacists would steer you away from it entirely. Please don't combine these or change anything on your own. Talk with your doctor or pharmacist first.
