Here's the honest picture with Midol Max Strength (acetaminophen, caffeine, and pyrilamine) alongside Puravive. The concern comes from the ginseng in Puravive, which may nudge a liver enzyme called CYP3A4 either up or down. The catch is that none of these three ingredients lean heavily on CYP3A4. Acetaminophen is mostly handled by other pathways, caffeine clears through CYP1A2, and pyrilamine is an antihistamine, so this particular interaction is pretty weak and mostly theoretical for this combo.
In plain terms, I wouldn't expect a big change in how this Midol works or how you feel. The evidence here is low quality and the contradictory research makes it hard to predict. Still, if you take this regularly or have liver concerns, run it by your pharmacist or doctor first.

That said, the evidence here is weak and this is rated as a minor, unlikely interaction. For most people it won't cause a real problem. The acetaminophen and pyrilamine parts of Midol aren't affected by ginseng. If you're sensitive to caffeine or have heart concerns, just run it by your pharmacist or doctor first.
