Macitentan, Tadalafil falls under Cytochrome P450 2C9 (Cyp2C9) Substrates — the group Puravive interacts with.

Puravive contains quercetin, which has been shown in studies to mildly inhibit the CYP2C9 metabolizing enzyme. This inhibition could potentially increase the risk of side effects from drugs. A small clinical trial with healthy volunteers found that taking 500 mg of quercetin twice daily for 10 days before using diclofenac, a drug metabolized by CYP2C9, increased diclofenac plasma levels by 75% and extended its half-life by 32.5% (source: https://pubmed.ncbi.nlm.nih.gov/29168292/). Combining quercetin with drugs metabolized by CYP2C9 could increase the risk of side effects, so caution is advised.

Here's the situation with Opsynvi, which pairs macitentan (a blood pressure medicine for the lungs) with tadalafil (the same drug family as Cialis). Puravive contains quercetin, and studies suggest quercetin can mildly slow down a liver enzyme called CYP2C9. Both parts of Opsynvi rely partly on this enzyme system to be cleared from your body, so in theory the levels could creep up a bit.

If that happened, you might notice more of the usual side effects: headache, flushing, a stuffy nose, dizziness, or lightheadedness when standing, since both ingredients can lower blood pressure. Honestly, the evidence here is limited and comes from a small quercetin study using a different drug, so this is more caution than alarm. Please run it by your pharmacist or doctor before combining them.

Macitentan, Tadalafil also falls under Cytochrome P450 3A4 (Cyp3A4) Substrates.

Puravive contains Panax ginseng. Panax ginseng has the potential to affect the breakdown of drugs that are metabolized by an enzyme called CYP3A4. One example of such a drug is imatinib. There was a case where imatinib was thought to have caused liver damage, and it was believed that this might have been due to Panax ginseng inhibiting CYP3A4, which is involved in imatinib's metabolism (https://pubmed.ncbi.nlm.nih.gov/20332334/). On the other hand, Panax ginseng has been found to increase the breakdown of another drug called midazolam, which is also metabolized by CYP3A4 (https://pubmed.ncbi.nlm.nih.gov/21646440/), suggesting Panax ginseng induces CYP3A4 in some cases (i.e., makes more potent). Given these contradictory findings, it is important to use Panax ginseng cautiously if you are taking medications that are metabolized by CYP3A4, as the interaction between Panax ginseng and these drugs is not fully understood.

Opsynvi combines two medicines, macitentan and tadalafil, and both are cleared partly by the liver enzyme CYP3A4. Puravive contains Panax ginseng, which in some studies seemed to slow that enzyme and in others sped it up. So honestly, the direction here isn't clear cut. If ginseng slowed the breakdown, blood levels could edge up and you might notice more headache, flushing, low blood pressure, dizziness, or nasal congestion. If it sped things up, the medicine could work a bit less well for your pulmonary hypertension.

The good news is this is rated a minor interaction based on weak, mixed evidence, not solid proof with this exact combo. Still, since you're managing a serious heart and lung condition, I'd run Puravive past your pharmacist or doctor before starting it.

Puravive contains quercetin, which has been found to slightly reduce blood pressure in individuals with mild hypertension (https://pubmed.ncbi.nlm.nih.gov/17951477/). There is a theoretical possibility that it could enhance the blood pressure-lowering effects of antihypertensive medications.

Opsynvi combines two medicines (macitentan and tadalafil) that both work partly by relaxing blood vessels, so it already tends to lower your blood pressure. Puravive contains quercetin, which in one small study nudged blood pressure down a little in people with mild high blood pressure. In theory, stacking quercetin on top of Opsynvi could add to that blood-pressure-lowering effect.

If your pressure drops too far, you might feel dizzy, lightheaded, flushed, or a bit off when standing up quickly. Honestly, this is a minor and mostly theoretical concern, and everyday food amounts of quercetin are unlikely to matter much. Still, because Opsynvi is a serious medication, please check with your pharmacist or doctor before adding Puravive.
