Why this applies to Crizotinib

Crizotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the gist for Xalkori (crizotinib). Your body relies on a liver enzyme called CYP3A4 to break this cancer medicine down. In lab dish studies, aegeline appears to block that same enzyme. If that slowed-down clearance carried over into people, crizotinib levels could drift upward, which might mean more of its usual side effects like nausea, vision changes, diarrhea, swelling, fatigue, or strain on the liver and heart rhythm.

I want to be honest about the strength here: this is only test-tube and animal data, not something seen in real patients, so it's theoretical. That said, aegeline itself has been tied to serious liver injury and pulled from products, so I'd steer clear of it entirely. Please loop in your oncology team or pharmacist before combining anything with your cancer therapy.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

About Crizotinib

What Is Crizotinib Used For?

Brand name: Xalkori

Xalkori (crizotinib) is used to treat adult patients with metastatic non-small cell lung cancer (NSCLC) whose tumors test positive for the ALK or ROS1 gene abnormality, as confirmed by an FDA-approved test.

Xalkori is also used to treat pediatric patients aged 1 year and older and young adults with relapsed or refractory systemic ALK-positive anaplastic large cell lymphoma (ALCL). Additionally, it treats adult and pediatric patients aged 1 year and older with unresectable, recurrent, or refractory ALK-positive inflammatory myofibroblastic tumor (IMT). Note: Xalkori's safety and effectiveness in older adults with relapsed or refractory ALK-positive ALCL have not been established.
