Why this applies to Verapamil

Verapamil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the gist for verapamil: your liver uses an enzyme called CYP3A4 to break this medicine down. In lab dish studies, aegeline appears to slow that same enzyme. If that held true in the body, verapamil could clear more slowly and build up, which might exaggerate its effects. With verapamil that could mean a slower heart rate, lower blood pressure, dizziness, lightheadedness, swollen ankles, or constipation.

The honest part: this is only a theoretical, test-tube finding, and it has never been reported in real people. On top of that, aegeline has been tied to serious liver injury, so it's a supplement I'd steer clear of anyway. Don't change your verapamil on your own. Loop in your pharmacist or doctor before mixing the two.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
