Why this applies to Modafinil

Modafinil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the basic idea: your body partly relies on a liver enzyme called CYP3A4 to clear modafinil. In lab dish studies, aegeline appears to slow that same enzyme down. If that were to happen in real life, modafinil could clear more slowly and build up a bit, which might mean stronger wakefulness effects or more of its usual side effects, like headache, jitteriness, a racing heartbeat, trouble sleeping, or anxiety.

I want to be honest about the strength of this, though. This is only seen in test-tube research, not in actual people, so the real-world risk is rated as minor and just theoretical. My bigger concern with aegeline is that it's been tied to serious liver injury and was pulled from products. I'd skip it and chat with your pharmacist first.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

What Is Modafinil Used For?

Brand names: Provigil, Sparlon, Alertec

Modafinil (also sold as Provigil) is used to improve wakefulness in adults who experience excessive sleepiness due to narcolepsy, obstructive sleep apnea (OSA), or shift work disorder (SWD).

In OSA, modafinil treats the sleepiness — not the underlying airway obstruction itself. It is used alongside standard treatments such as CPAP (a breathing device worn during sleep), not as a replacement for them. If CPAP is appropriate for you, your doctor will want you to use it consistently even while taking modafinil.
