Why this applies to Dextromethorphan

Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Aegeline interacts with.

Aegeline inhibits cytochrome P450 3A4 (CYP3A4) activity in vitro. So far, this interaction has not been reported in humans. Theoretically, aegeline may increase levels of drugs metabolized by CYP3A4. Some drugs metabolized by CYP3A4 include lovastatin (Mevacor), ketoconazole (Nizoral), itraconazole (Sporanox), fexofenadine (Allegra), triazolam (Halcion), and numerous others. Use aegeline cautiously or avoid in patients taking these drugs.

Here's the short version: your cough medicine, dextromethorphan, is partly broken down by a liver enzyme called CYP3A4. In test-tube studies, aegeline appears to slow that same enzyme down. In theory, that could let a bit more dextromethorphan hang around in your body, which might mean stronger effects like drowsiness, dizziness, or feeling foggy or nauseated.

I want to be honest with you, though: this is only lab data, not something seen in real people, and it's rated a minor interaction. The bigger concern by far is aegeline itself, which has been tied to serious liver injury and is one regulators have warned against. For that reason alone, I'd skip it and check with your pharmacist before combining the two.

Aegeline is a compound originally identified in the bael tree (Aegle marmelos) that gained attention because it was added to some weight-loss and workout supplements. It is best known for being linked to serious liver injury in a product called OxyELITE Pro, and U.S. regulators warned against products containing it. There is no good evidence it safely helps with weight loss, and it should generally be avoided.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
