Why this applies to Dextromethorphan

Dextromethorphan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: on paper, red clover's plant compounds (isoflavones) could nudge up the enzyme system (CYP3A4) that helps break down dextromethorphan. If that happened, more of your cough medicine could linger, which in theory might mean a bit more drowsiness, dizziness, or stomach upset. Dextromethorphan is active on its own (not a prodrug), so slower clearance would mean a slightly stronger effect, not a weaker one.

The good news is this is mostly theoretical. A small human study using a similar CYP3A4 medicine showed no real change, so this is rated minor and unlikely. For a short-term cough remedy, I wouldn't lose sleep over it, but if you take red clover daily, just mention it to your pharmacist or doctor.

Dextromethorphan temporarily relieves cough due to minor throat and bronchial irritation, such as the kind that comes with the common cold or breathing in irritants. It also helps reduce the urge to cough so you — or your child — can get to sleep.

Some products, including CVS Long Acting Cough and Premier Value Tussin Cough oral liquids, are also indicated to help loosen phlegm (mucus) and thin bronchial secretions to help drain the airways.
