Theoretically, red clover might increase the levels and clinical effects of drugs metabolized by CYP2C19.

Here's the gist for phenytoin (Dilantin) and red clover. Your liver uses an enzyme called CYP2C19 to help clear phenytoin. In a test-tube setting, red clover's plant compounds weakly slowed that same enzyme down. If that happened in your body, phenytoin could clear a little more slowly and build up, which in theory might mean more side effects like dizziness, drowsiness, unsteady walking, slurred speech, or jerky eye movements.

That said, this is very theoretical. It's only been seen in lab studies, never reported in real people, and the effect was weak. Still, phenytoin is a finicky drug where small shifts can matter, so before adding red clover, please check with your pharmacist or doctor first.

Phenytoin also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the gist for phenytoin (Dilantin) and red clover. In a test tube, red clover's isoflavones look like they might slow down CYP2C9, which is one of the liver enzymes that clears phenytoin. If that actually happened in your body, phenytoin levels could creep up, and since phenytoin has a narrow safety window, even small bumps can cause side effects like dizziness, drowsiness, unsteady walking, slurred speech, or wobbly eye movements.

The reassuring part: this is mostly lab-based thinking. A small human study using a similar CYP2C9 drug showed no real change, so the actual risk looks low. Still, phenytoin is a drug where I never like surprises. Please check with your pharmacist or doctor before adding red clover.

The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. About Phenytoin

What Is Phenytoin Used For?

Brand names: Dilantin Kapseals, Dilantin, Di-Phen, Dilantin Infatabs, Phenytek, Dilantin injection

Phenytoin is used to treat and prevent seizures. The specific types covered depend on the formulation. Oral extended-release capsules (including Phenytek), chewable tablets (including Dilantin Infatabs and Phenytoin Infatabs), and oral suspension (including Dilantin-125) are used for tonic-clonic (grand mal) and psychomotor (temporal lobe) seizures; most oral forms also cover prevention and treatment of seizures occurring during or after neurosurgery. Chewable tablets and some capsule formulations also cover complex partial seizures.

Parenteral (injectable) phenytoin sodium is used for generalized tonic-clonic status epilepticus (a prolonged, life-threatening seizure emergency) and for prevention and treatment of seizures during neurosurgery. The IV form can also substitute short-term for oral phenytoin when a patient cannot take it by mouth.
