Why this applies to Theophylline
Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for theophylline: your liver clears it using an enzyme called CYP1A2. In a test tube, red clover's plant estrogens (isoflavones) seem to slow that enzyme down a bit. If that happened in real life, theophylline could build up and push you toward its known side effects, things like a racing heart, jitteriness, nausea, trouble sleeping, or even tremors. That matters because theophylline is a touchy drug where the helpful dose and the too-much dose sit close together.
The reassuring part: when researchers actually tested red clover in people, it did not change how the body handled a similar CYP1A2 substrate. So this concern is mostly theoretical. Still, since theophylline is finicky, it's worth a quick word with your pharmacist or doctor before adding red clover.

Theophylline — available as extended-release capsules (Theo-24), extended-release tablets, and oral solution (Elixophyllin) — is used to treat symptoms and reversible airflow obstruction caused by chronic asthma, emphysema, and chronic bronchitis.
All supported formulations share these indications. Theophylline is intended for patients with ongoing or recurring symptoms, not for sudden (acute) breathing attacks, which need faster-acting treatments.
