Why this applies to Propranolol

Propranolol falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the short version: your body partly clears propranolol using a liver enzyme called CYP1A2. In a test tube, red clover's plant estrogens can slow that enzyme down a little. If that happened in real life, propranolol could build up slightly, which in theory might mean a bit more of its usual effects, like a slower heart rate, lower blood pressure, lightheadedness, or tiredness.

The reassuring part is that this is mostly lab-dish thinking. When researchers actually tested red clover isoflavones in people, they saw no real change in how a CYP1A2 drug was handled. So this is rated minor and unlikely. Still, if you notice unusual dizziness or fatigue after starting red clover, check in with your pharmacist or doctor.

In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.

Here's the gist for propranolol and red clover. In a lab dish, red clover slightly slowed down a liver enzyme called CYP2C19, which is one of the pathways that helps clear propranolol from your body. In theory, that could let a bit more propranolol build up, which might nudge its effects, like a slower heart rate, lower blood pressure, lightheadedness, or feeling more tired, a little stronger.

But I want to keep this in perspective. This is only a test-tube finding, it has never actually been seen in real people, and propranolol mostly relies on a different enzyme anyway. Food-amount soy or clover is very unlikely to matter; concentrated supplements are the bigger question mark. If you'd like to use it, just run it by your pharmacist or doctor first.

The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Propranolol (Cardiovascular) Used For?

Brand names: Inderal LA, Inderal, Angilol, Syprol, Inderal Injection, Half-Inderal LA, Bedranol SR, Half Beta Prograne, Beta Prograne, Lopranol LA, Slo-Pro, InnoPran XL

This intravenous (IV) form of propranolol is used in hospital or clinical settings to treat dangerous heart rhythm problems. It is generally reserved for life-threatening arrhythmias (abnormal heart rhythms) or those that occur during anesthesia.

Specific conditions it treats include fast heart rates coming from the upper chambers of the heart (supraventricular tachycardia), certain arrhythmias caused by digitalis toxicity, dangerous ventricular rhythms that haven't responded to other treatments, and fast heart rates caused by excess adrenaline-like substances during surgery.
