Hydrocodone, Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the lowdown on combining red clover with Vicoprofen, which pairs hydrocodone with ibuprofen. The ibuprofen half is handled in part by a liver enzyme called CYP2C9. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let ibuprofen levels creep up and bring more of the usual side effects (stomach upset, heartburn, or kidney strain over time).

The good news: when researchers actually tested standardized red clover in people, it did not change how a CYP2C9 drug was processed. So this is a theoretical, lab-only concern that real-world studies haven't backed up. I wouldn't lose sleep over it, but since you're on an opioid combo, it's always smart to run your supplement list past your pharmacist or doctor.

The hydrocodone part of Vicoprofen is partly handled by a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let hydrocodone build up a bit (think extra drowsiness, dizziness, constipation, or slowed breathing). But here's the reassuring part: when researchers actually tested red clover in people using a similar CYP3A4 drug, they saw no real change.

So this is mostly a theoretical, lab-based concern, and the real-world risk looks low. I still wouldn't ignore it, since hydrocodone is an opioid. If you take Vicoprofen regularly and want to try red clover, just run it by your pharmacist or doctor first to be safe.

ANTICOAGULANT/ANTIPLATELET DRUGS

Hydrocodone, Ibuprofen also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the practical picture for Vicoprofen, which combines hydrocodone (an opioid pain reliever) with ibuprofen (an NSAID). The concern with red clover is a theoretical one: some lab studies hint that its isoflavones, like genistein, might thin the blood a little or slow platelets. The ibuprofen half of Vicoprofen already nudges platelets in that direction, so in theory the two could add up and raise bleeding risk, showing up as easy bruising, stomach upset, or dark stools.

The good news is that actual human studies didn't find this blood-thinning effect from red clover, so the real-world risk looks low. Still, if you take Vicoprofen regularly or have any stomach or bleeding issues, it's worth a quick chat with your pharmacist before adding red clover.
