Estratest contains real estrogen along with a form of testosterone, and it's often prescribed for menopause symptoms like hot flashes. Red clover carries phytoestrogens, plant compounds that loosely mimic your body's own estrogen. The concern here is mostly on paper: in theory, those plant estrogens could crowd in at the same receptors your medication is trying to reach, possibly nudging how well your estrogen therapy works.

To be clear, this is a theoretical interaction based on how the compounds behave, not on strong human studies, and small food amounts of red clover are unlikely to matter. Concentrated supplement doses are where I'd pay attention. Since both products affect hormones, it's worth a quick chat with your pharmacist or doctor before combining them rather than changing anything on your own.

Esterified Estrogens, Methyltestosterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version on pairing red clover with Estratest (esterified estrogens plus methyltestosterone). In a test tube, red clover looked like it might slow down the CYP3A4 enzyme, the same liver pathway that helps break down the estrogen part of this medicine. If that slowdown happened in real life, hormone levels could theoretically creep up, which might mean more breast tenderness, bloating, nausea, or spotting.

But here's the reassuring part: when researchers actually tested standardized red clover capsules in people, they found no meaningful change in a CYP3A4 drug. So this is mostly theoretical and rated minor and unlikely. One extra wrinkle worth a quick chat with your pharmacist: red clover has weak estrogen-like activity of its own, so combining hormones is still worth mentioning to your doctor.
