Doxorubicin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version on combining red clover with doxorubicin. In a lab dish, red clover's isoflavones looked like they might slow down CYP3A4, one of the liver enzymes that helps break down doxorubicin. If that happened in real life, more of the drug could linger in your body, which in theory could mean stronger side effects like nausea, mouth sores, low blood counts, or heart strain.

But here's the reassuring part: when red clover was actually tested in people using a standard CYP3A4 test drug, it didn't change those levels at all. So this concern stays mostly theoretical. Still, doxorubicin is serious chemotherapy, so please run any supplement past your oncology team first. They may also prefer you avoid red clover's estrogen-like activity during treatment.

Doxorubicin is used to treat a wide range of cancers. It is given as part of combination chemotherapy for women with breast cancer that has spread to the lymph nodes after surgery, and it is used for many other cancers — from blood cancers like leukemia and lymphoma to solid tumors like bone and soft tissue sarcomas.

It is considered a broad-spectrum cancer fighter, meaning it is active against many different tumor types. It is almost always given alongside other chemotherapy drugs rather than on its own.
