ESTROGENS

Ethinyl Estradiol, Norgestimate falls under ESTROGENS — the group Red Clover interacts with.
Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Ortho Tri-Cyclen and its generics (Sprintec, Mononessa, and others) combine ethinyl estradiol with norgestimate to prevent pregnancy and help regulate your cycle. Red clover carries phytoestrogens, plant compounds that loosely resemble estrogen. The thought here is that, in larger supplement-sized amounts, these compounds could compete at the same estrogen receptors your pill is working on and slightly blunt or shift its hormonal effect.
I want to be honest about how soft this concern is: it's theoretical, drawn from how these compounds behave in the lab, not from real-world failures of birth control. Still, since reliable contraception matters, it's worth running by your pharmacist or doctor before you start red clover, rather than guessing on your own.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the short version on combining red clover with your birth control pill (the ethinyl estradiol and norgestimate combo). In a test tube, red clover slows down an enzyme called CYP1A2, which is one of the pathways your body uses to clear certain medications. In theory, blocking that enzyme could let a drug build up a little. So the worry would be slightly higher estrogen levels, which can mean things like nausea, breast tenderness, or spotting.
But here's the reassuring part: this concern comes only from lab data, and an actual human study found no real effect on a CYP1A2 marker. So a meaningful interaction is unlikely. Red clover also has its own weak estrogen-like activity, which is a separate reason to check in. I'd still run it by your pharmacist or doctor before starting it.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist for your birth control pill (Ortho Tri-Cyclen and similar brands): the estrogen part, ethinyl estradiol, is broken down by a liver enzyme called CYP3A4. In a lab dish, red clover looked like it might slow that enzyme down a little, which in theory could nudge your pill's hormone levels up. If that happened, you might notice more estrogen-type side effects like nausea, breast tenderness, or headaches.
The reassuring news is that when researchers actually tested red clover in people, it did not change how the body handled a similar CYP3A4 medication. So this is mostly a theoretical, lab-based concern, and it's rated minor and unlikely.
