Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
The Ortho Evra patch delivers two hormones, an estrogen (ethinyl estradiol) and a progestin (norelgestromin), to prevent pregnancy and steady your cycle. Red clover carries phytoestrogens, plant compounds that loosely mimic estrogen and can sit on the same receptors your body uses. In theory, large supplement-sized doses could nudge how your patch's estrogen works, either competing with it or adding a small extra estrogen-like push.
Honestly, this concern is theoretical and based on how the chemistry looks, not on solid human studies. There is no good evidence it weakens your birth control or causes real problems. Still, since this is a hormone-based product, it's worth a quick chat with your pharmacist or doctor before adding red clover, rather than guessing.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

If you're wearing the Ortho Evra (Evra) patch and thinking about red clover for hot flashes, here's the honest picture. In lab dishes, red clover can mildly slow down an enzyme called CYP1A2, which plays a small role in handling the estrogen part of your patch. In theory, slowing that enzyme could let estrogen levels creep up a bit, which might mean more nausea, breast tenderness, or bloating.
But here's the reassuring part: this is only test-tube evidence, and a small human study showed red clover did not actually change how the body cleared a similar CYP1A2 substrate. So a real-world problem is unlikely. Since red clover has estrogen-like activity of its own, it's still worth a quick chat with your pharmacist or doctor before adding it.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. If you're using the Ortho Evra (Evra) patch, which delivers norelgestromin and ethinyl estradiol, here's the gist. In lab dishes, red clover looked like it might slow down CYP3A4, the liver enzyme that helps clear the estrogen part of your patch.
