Here's the honest picture with red clover and your formoterol inhaler. Red clover has plant compounds that, in a test tube, very weakly slow down a liver enzyme called CYP2C19. The theory is that blocking that enzyme could let drugs broken down by it build up a little. But formoterol isn't really a CYP2C19 drug in any meaningful way, and it's an active medication, not a prodrug that needs switching on, so this pathway just isn't a big player for it.

The bottom line: this is lab-only, theoretical, and has never been reported in real people. I wouldn't expect more jitteriness, fast heartbeat, or shakiness from this combo. Still, since red clover acts like a weak estrogen, run it by your pharmacist or doctor first, especially with any hormone-sensitive condition.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: this one barely registers. The concern is that red clover's plant compounds (isoflavones) might slow down a liver enzyme called CYP2C9, which could let certain drugs build up. The catch is that formoterol, your long-acting inhaler for COPD, is mostly cleared a different way and isn't really driven by CYP2C9, so this pairing is unlikely to cause trouble.

On top of that, the evidence here is weak. It comes from lab dish studies, and a small human study found no meaningful effect. So I wouldn't expect red clover to make formoterol act stronger or cause more shakiness, a racing heart, or jitteriness. Still, since red clover has hormone-like effects, run it past your pharmacist or doctor before starting it.

What Is Formoterol Oral Inhalation Used For?

Brand names: Foradil Aerolizer, Atimos Modulite, Oxis Turbohaler, Perforomist, Foradil Certihaler

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
