Verapamil falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the gist for verapamil and red clover. In a test tube, red clover can slow down a liver enzyme called CYP1A2. The thinking is that if that enzyme runs slower, a drug it helps clear could build up a little. Since verapamil works as-is (it isn't a prodrug that needs activating), higher levels would mean more of its usual effects, like extra-low blood pressure, dizziness, a slower pulse, or constipation.
But honestly, I wouldn't lose sleep over this one. The lab signal didn't hold up in people: a small human study found red clover didn't change how the body handled a CYP1A2 test drug. It's also worth noting verapamil leans more on a different enzyme (CYP3A4). Still, mention it to your pharmacist so they can keep an eye on things.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist: your body clears verapamil mainly through a liver enzyme called CYP3A4. In a test-tube setting, red clover's plant estrogens looked like they might slow that enzyme down. If that happened in real life, verapamil could build up a bit, which in theory might mean more of its usual effects, like lower blood pressure, dizziness, a slower heartbeat, or constipation.
But here's the reassuring part: an actual human study using a similar CYP3A4 medication showed no real change. So this concern stays mostly theoretical. Verapamil is not a prodrug, so it doesn't flip the other way. Still, since verapamil affects your heart rate and blood pressure, it's worth a quick word with your pharmacist or doctor before adding red clover.

The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. What Is Verapamil Used For?
Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil
Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.
Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
