Your birth control pill combines an estrogen (ethinyl estradiol) with a progestin (levonorgestrel). Red clover carries plant compounds called phytoestrogens that act like a very weak version of estrogen. The concern here is purely theoretical: in large supplement doses, those phytoestrogens could in principle nudge at the same estrogen receptors your pill uses, possibly tugging on its hormonal balance.

I want to be honest, this is based on how the chemistry looks on paper, not on real-world reports of pills failing. Casual or occasional use likely matters little. Still, since this pill prevents pregnancy and steady hormone levels matter, it's worth a quick chat with your pharmacist or doctor before starting concentrated red clover, rather than guessing on your own.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the gist for your birth control pill (the levonorgestrel and ethinyl estradiol combo). In a test tube, red clover slowed down an enzyme called CYP1A2, which plays a small role in handling certain medications. In theory, blocking it could let drug levels creep up a little. But here's the reassuring part: in an actual study in people, red clover did not change how the body cleared a CYP1A2 test substance.

So for your pill, this is a very weak, mostly theoretical concern. Your pill isn't a prodrug, and CYP1A2 isn't its main pathway anyway, so a meaningful change in side effects (like nausea or breast tenderness) is unlikely. Still, since red clover has mild estrogen-like activity, it's worth a quick chat with your pharmacist or doctor before starting it.

If you take a combined birth control pill like Microgynon or Ovranette and you're curious about red clover for hot flashes, here's the honest picture. In a lab dish, red clover's isoflavones looked like they might slow down CYP3A4, the liver enzyme that helps clear the estrogen part (ethinyl estradiol) of your pill. In theory, that could nudge hormone levels up a little, which might mean more nausea, breast tenderness, or spotting.

But in real people, a study using a CYP3A4 test drug showed no meaningful change, so this is mostly theoretical and rated minor and unlikely. Your pill isn't a prodrug, so it doesn't get flipped. Still, since estrogen-sensitive concerns matter here, run red clover past your pharmacist or doctor first.
