Nortriptyline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the gist for these two together. Your body uses a liver enzyme called CYP1A2 to help clear nortriptyline. In a test tube, red clover can slow that enzyme down a bit, and the worry is that nortriptyline could pile up and cause more of its usual side effects, like dry mouth, constipation, drowsiness, blurred vision, or a faster heartbeat.

The good news is this concern is mostly theoretical. When researchers actually gave red clover to people, it didn't meaningfully change how the body handled a CYP1A2 test drug. So a real-world problem here is unlikely. Still, if you notice new grogginess or other side effects after adding red clover, check in with your pharmacist or doctor before changing anything.
