Apalutamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

If you're taking Erleada (apalutamide) for prostate cancer and wondering about red clover, here's the honest picture. The concern on paper is that red clover's isoflavones might block CYP3A4, a liver enzyme that helps clear apalutamide. In theory, slowing that breakdown could let levels creep up, which might add to side effects like fatigue, rash, joint pain, high blood pressure, or diarrhea.

But the real-world evidence is reassuring. When researchers tested a standardized red clover dose in people, it did not meaningfully change how the body handled a CYP3A4 test drug. So this interaction is mostly theoretical and rated minor and unlikely. There's a separate point worth raising though: red clover acts like a weak plant estrogen, and that hormone activity is something to discuss with your oncology team before starting it.

What Is Apalutamide Used For?

Brand name: Erleada

Erleada (apalutamide) is used to treat two types of prostate cancer. The first is metastatic castration-sensitive prostate cancer (mCSPC) — prostate cancer that has spread to other parts of the body and still responds to hormone-lowering therapy. The second is non-metastatic castration-resistant prostate cancer (nmCRPC) — prostate cancer that has stopped responding to standard hormone therapy but has not yet spread to other organs.

Erleada is always used alongside a gonadotropin-releasing hormone (GnRH) analog — a medicine that lowers testosterone — or in patients who have already had surgery to remove the testicles.
