Red Clover and Estradiol, Progesterone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Red Clover interacts with whole categories of medication. Estradiol, Progesterone falls into 3 of those categories, so it can interact with Red Clover in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ESTROGENS

1 of 3

Why this applies to Estradiol, Progesterone

Estradiol, Progesterone falls under ESTROGENS — the group Red Clover interacts with.

Theoretically, concomitant use of large amounts of red clover might interfere with estrogen therapy.
Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Here's the idea behind this one. Bijuva gives you real estradiol (estrogen) plus progesterone to ease menopause symptoms like hot flashes and night sweats. Red clover contains plant compounds called phytoestrogens that can weakly attach to the same estrogen receptors your medication is trying to activate.

In theory, taking large, concentrated amounts of red clover could compete with your estradiol and slightly blunt how well it works, meaning your hot flashes or other symptoms might not be controlled as well. I'll be honest: this is theoretical, based on how the plant behaves in the lab, not on strong patient studies. Still, since both act on hormone pathways, I'd skip the red clover supplement and let your prescription do its job. Please check with your pharmacist or doctor first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Estradiol, Progesterone also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Bijuva (estradiol plus progesterone) and red clover. In test-tube studies, red clover slowed down a liver enzyme called CYP1A2. Estradiol is partly handled by that same enzyme, so in theory red clover could let a little more estradiol linger in your system. If that happened, you might notice things like breast tenderness, bloating, nausea, or spotting.

But here's the reassuring part: when researchers actually tested red clover in people, it didn't change how the body cleared a CYP1A2 marker drug. So this is mostly a lab-bench worry, not something seen in real patients. Since red clover itself has estrogen-like activity, it's still worth a quick chat with your pharmacist or doctor before combining the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Estradiol, Progesterone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body clears the hormones in Bijuva (estradiol and progesterone) partly through a liver enzyme called CYP3A4. In lab dishes, red clover looks like it might slow that enzyme down a little, which in theory could let these hormones build up slightly and nudge up side effects like breast tenderness, bloating, headaches, or spotting.

But here's the reassuring part: when researchers actually tested red clover in real people, it did not meaningfully change how a CYP3A4-cleared drug was processed. So this concern stays mostly theoretical and unlikely to cause trouble. Since red clover also acts like a weak plant estrogen, the bigger conversation is whether it's right for you alongside hormone therapy. Please run it by your pharmacist or doctor first.

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.
