Nifedipine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Your body relies on a liver enzyme called CYP3A4 to clear nifedipine. In a test tube, red clover looked like it might slow that enzyme down a bit. If that happened in real life, more nifedipine could linger in your system, which in theory might lead to lower blood pressure, dizziness, flushing, headache, or some ankle swelling.

The good news is that this concern is mostly theoretical. When researchers gave people standardized red clover isoflavones twice a day for two weeks, it didn't change how the body handled a similar CYP3A4 medication. So a meaningful interaction here is unlikely. Still, it's smart to mention red clover to your pharmacist or doctor, especially if you notice new lightheadedness after starting it.

What Is Nifedipine Used For?

Brand names: Procardia XL, Adalat PA, Adalat, Nifedin, Nifed PA, Nifed, Procardia, Adalat XL, Adalat CC, Adalat LA, Adalat Retard, Adipine MR, Adipine XL, Cardilate MR, Coracten SR, Coracten XL, Fortipine LA 40, Hypolar Retard 20, Nifedipress MR, Nifopress Retard, Tensipine MR

Nifedipine treats high blood pressure (hypertension), chronic stable angina (chest pain brought on by physical effort), and vasospastic angina (chest pain caused by spasm of the coronary arteries, sometimes called Prinzmetal's angina). Lowering blood pressure with nifedipine helps reduce the risk of strokes and heart attacks.

The capsule form is approved for vasospastic angina and chronic stable angina only — it is not approved for high blood pressure. The extended-release tablet (including Procardia) covers all three uses: vasospastic angina, chronic stable angina, and high blood pressure.
