Norethindrone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the gist: norethindrone is a progestin that your liver clears partly through an enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let a bit more norethindrone build up. If that happened, you might notice more of its usual effects, like breast tenderness, bloating, mood changes, or spotting between periods.

But here's the reassuring part. When researchers actually tested red clover isoflavones in people, it didn't meaningfully change how the body handled a CYP3A4 drug. So this concern stays mostly theoretical and is rated minor and unlikely. Norethindrone is not a prodrug, so it doesn't flip. Still, since red clover has weak estrogen-like activity, it's worth a quick check with your pharmacist or doctor before combining them.

What Is Norethindrone Used For?

Brand names: Aygestin, Norlutin, Nor-QD, Micronor, Norlutate, Camila, Errin, Jolivette, Nora-BE

Norethindrone is a progestin-only oral contraceptive used to prevent pregnancy. It is taken every day, continuously, without hormone-free breaks — unlike combination birth control pills.

Gallifrey (norethindrone acetate tablets, 5 mg) is a different, higher-dose formulation approved for three additional conditions: secondary amenorrhea (periods that have stopped), endometriosis, and abnormal uterine bleeding due to hormonal imbalance when no structural cause such as fibroids or uterine cancer is present. Gallifrey is not approved or recommended for use alongside estrogen therapy in postmenopausal women for endometrial protection.
