Red Clover and Aspirin, Omeprazole

These two interact in 4 distinct ways. Each one is explained separately below.

Why Are There 4 Interactions?

Red Clover interacts with whole categories of medication. Aspirin, Omeprazole falls into 4 of those categories, so it can interact with Red Clover in 4 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Minor).

Minor

CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES

1 of 4

Why this applies to Aspirin, Omeprazole

Aspirin, Omeprazole falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase the levels and clinical effects of drugs metabolized by CYP2C19.
In vitro evidence suggests that red clover weakly inhibits CYP2C19. This interaction has not been reported in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Here's the situation with Yosprala (a combo pill of aspirin and omeprazole) and red clover. The omeprazole part is partly cleared by a liver enzyme called CYP2C19. In a test-tube setting, red clover's plant compounds (isoflavones) weakly slowed that enzyme down. In theory, that could let a little more omeprazole linger in your body. Since omeprazole is already very well tolerated, this would most likely mean nothing noticeable, though in rare cases people get things like headache, nausea, or loose stools.

Honestly, this is a theoretical, lab-only finding that has never been seen in real people, and it's rated minor and unlikely. The aspirin side isn't really affected here. Still, since red clover acts like a weak estrogen, it's worth a quick check with your pharmacist or doctor before starting it.

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

2 of 4

Aspirin, Omeprazole also falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: this one is mostly theoretical. Lab (test tube) studies hint that red clover's isoflavones could slow down a liver enzyme called CYP2C9, which helps clear part of this medicine. If that happened, you might in theory see a touch more aspirin effect, like easier bruising, stomach upset, or minor bleeding. But a real human study using a CYP2C9 test drug found no meaningful change, so the actual risk looks very low.

Bottom line: for most people, normal red clover use alongside your Yosprala (aspirin and omeprazole) is unlikely to cause a problem. Still, since aspirin already affects bleeding, it's smart to mention any new supplement to your pharmacist or doctor before starting.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 4

Aspirin, Omeprazole also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: this one is mostly theoretical, and you don't need to lose sleep over it. The thought is that red clover's plant compounds could slow down a liver enzyme called CYP3A4, which might let some medications build up a bit. The omeprazole part of Yosprala is partly handled by that enzyme, so in theory levels could nudge upward.

But the real-world evidence is reassuring. When researchers gave people a standardized red clover dose for two weeks, it did not meaningfully change how a sensitive CYP3A4 drug was processed. So a real, troublesome interaction here is unlikely. Aspirin isn't really affected by this pathway anyway. Still, since red clover acts like a weak estrogen, it's worth a quick chat with your pharmacist before starting it.

ANTICOAGULANT/ANTIPLATELET DRUGS

4 of 4

Aspirin, Omeprazole also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: this is a pretty theoretical concern. Your medication pairs aspirin (a blood thinner that keeps platelets from clumping) with omeprazole (which calms stomach acid to protect against ulcers). Red clover has isoflavones like genistein, and in lab dishes those have shown some mild platelet-slowing activity. On paper, stacking that on top of aspirin could nudge you toward a little more bruising or bleeding.

But the real-world studies don't back this up. When postmenopausal women took red clover for up to two years, their clotting tests didn't budge. So the practical risk is low. Still, since aspirin already affects bleeding, keep an eye out for things like easy bruising, nosebleeds, or dark stools, and check with your pharmacist before adding red clover.

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.
