Diclofenac Sodium falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the gist for diclofenac: your liver uses an enzyme called CYP2C9 to clear it from your body. Red clover's plant compounds (isoflavones) have shown, in test-tube studies only, a slight ability to slow that enzyme down. In theory, that could let diclofenac build up a bit, which might mean more of its usual side effects like stomach upset, heartburn, or fluid retention.

But here's the reassuring part: this is theoretical and based on lab work, not real people. When researchers actually gave folks red clover and tracked a similar CYP2C9 medicine, they saw no meaningful change. So a real-world problem is unlikely. Still, it's worth a quick mention to your pharmacist or doctor, especially if you take diclofenac regularly.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the short version on combining red clover with diclofenac (Voltaren): the worry on paper is that both might thin the blood a little. Diclofenac is an NSAID, and like other NSAIDs it can affect platelets and irritate the stomach lining, which can nudge up your risk of bruising or bleeding. Red clover's isoflavones (like genistein) have shown some clot-slowing activity in lab dishes, so in theory the two together could add up.

In real life, though, this looks very unlikely to cause problems. Human studies giving red clover for months to years found no meaningful change in blood clotting. I'd still keep an eye out for things like unusual bruising, nosebleeds, or dark stools, and check with your pharmacist or doctor before starting red clover.
