Red Clover and Ethinyl Estradiol, Ethynodiol Diacetate

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Red Clover interacts with whole categories of medication. Ethinyl Estradiol, Ethynodiol Diacetate falls into 3 of those categories, so it can interact with Red Clover in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

ESTROGENS

1 of 3

Why this applies to Ethinyl Estradiol, Ethynodiol Diacetate

Ethinyl Estradiol, Ethynodiol Diacetate falls under ESTROGENS — the group Red Clover interacts with.

Theoretically, concomitant use of large amounts of red clover might interfere with estrogen therapy.
Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your pill, which pairs ethinyl estradiol with ethynodiol diacetate, is a combination birth control pill that also helps with cycle control. Red clover carries phytoestrogens, plant compounds that loosely resemble estrogen and can nudge the same receptors your pill works on. In theory, taking large, concentrated red clover doses could compete at those receptors and slightly muddy how your estrogen-containing pill behaves.

Here's the honest part: this concern is theoretical, based on how these compounds look on paper, not on solid studies showing real problems. Small food amounts are unlikely to matter. Still, since this pill helps prevent pregnancy and steady your hormones, it's worth a quick chat with your pharmacist or doctor before adding a red clover supplement.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Ethinyl Estradiol, Ethynodiol Diacetate also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with your birth control pill (Demulen, Zovia, Kelnor) and red clover. In lab dishes, red clover slows down an enzyme called CYP1A2, which helps process the estrogen part of your pill. In theory, blocking that enzyme could let estrogen levels creep up a bit, which might mean more of the usual estrogen side effects like breast tenderness, nausea, bloating, or headaches.

The good news: this is mostly a test-tube finding. When researchers actually gave people red clover, it did not meaningfully change how the body handled a CYP1A2 test drug. So a real-world problem here is unlikely. Still, since red clover acts like a weak plant estrogen and you're already taking estrogen, it's worth a quick chat with your pharmacist or doctor before starting it.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Ethinyl Estradiol, Ethynodiol Diacetate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your birth control pill (ethinyl estradiol with ethynodiol diacetate) is partly broken down by a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down a bit, which in theory could let estrogen levels climb a little. If that happened, you might notice more of estrogen's usual nuisances, like breast tenderness, nausea, or bloating.

But here's the reassuring part: when researchers actually tested red clover in people, it did not change how the body handled a CYP3A4 drug. So this is mostly a theoretical concern, rated minor and unlikely. One extra thought worth raising with your doctor is that red clover acts like a weak plant estrogen, so it's a good idea to check in before combining the two.

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.
