Estradiol, Medroxyprogesterone falls under ESTROGENS — the group Red Clover interacts with.
Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Divitren combines estradiol (a form of estrogen) with medroxyprogesterone, and it's usually prescribed to manage menopause symptoms or protect the uterine lining. Red clover is interesting here because its isoflavones act like weak plant estrogens. The theory is that these plant compounds could crowd in on the same estrogen receptors your prescribed estradiol is meant to reach, potentially blunting or muddying its effect.
I want to be honest that this is a theoretical concern based on how the chemistry works, not something proven in studies. It's also more relevant with concentrated red clover supplements than with tiny food amounts. If you're considering red clover while on Divitren, please check with your pharmacist or doctor first before changing anything.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the honest picture with red clover and your estradiol/medroxyprogesterone combo. In a lab dish, red clover's isoflavones can slow down a liver enzyme called CYP1A2, which plays a small part in clearing estrogen. In theory, that could let a bit more estrogen hang around, which might mean side effects like breast tenderness, bloating, nausea, or spotting. But this is only test-tube data, and when researchers actually gave people red clover, it did not change how the body handled a CYP1A2 test drug.
So the real-world risk here looks low. One extra wrinkle: red clover acts like a weak plant estrogen on its own, so stacking it on hormone therapy is worth a quick chat. Please run it by your pharmacist or doctor before adding it.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. Here's the practical picture with Divitren (estradiol plus medroxyprogesterone) and red clover.
