Red Clover and Ethinyl Estradiol, Norethindrone

These two interact in 3 distinct ways. Each one is explained separately below.

Why Are There 3 Interactions?

Red Clover interacts with whole categories of medication. Ethinyl Estradiol, Norethindrone falls into 3 of those categories, so it can interact with Red Clover in 3 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

ESTROGENS

1 of 3

Why this applies to Ethinyl Estradiol, Norethindrone

Ethinyl Estradiol, Norethindrone falls under ESTROGENS — the group Red Clover interacts with.

Theoretically, concomitant use of large amounts of red clover might interfere with estrogen therapy.
Red clover contains phytoestrogens which might have estrogenic activity in some people. Theoretically, red clover might compete for estrogen receptors and interfere with estrogen-containing drug therapy.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

The pills you're asking about combine ethinyl estradiol (an estrogen) with norethindrone, and they work by keeping your hormone levels in a steady, predictable range. Red clover carries plant compounds called phytoestrogens that can weakly latch onto the same estrogen receptors in your body. In theory, taking large, concentrated amounts of red clover could nudge that hormone balance, either by competing with the estrogen in your medicine or adding to it.

Honestly, this concern is theoretical and comes from how these compounds behave in the lab, not from strong human studies. The plant estrogens are also much weaker than what's in your pill. Still, since steady hormone levels matter here, I'd run it by your pharmacist or doctor before starting red clover rather than guessing.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

2 of 3

Ethinyl Estradiol, Norethindrone also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your birth control pill (ethinyl estradiol with norethindrone). In a test tube, red clover can slow down an enzyme called CYP1A2, which helps your body break down certain drugs. The thinking goes that if that enzyme is blocked, a little more medication could hang around. But this is mostly lab-dish data, and a small human study found that red clover did not actually change how the body handled a CYP1A2 test drug.

Bottom line: this is a theoretical, minor, and unlikely interaction. In real life, your pill is mostly cleared by other pathways, so any bump in estrogen-type side effects (nausea, breast tenderness, headaches) would likely be small if it happens at all. Since red clover has its own estrogen-like activity, it's still worth a quick chat with your pharmacist or doctor before combining the two.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

3 of 3

Ethinyl Estradiol, Norethindrone also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, red clover might increase levels of drugs metabolized by CYP3A4; however, a small clinical study found no effect.
In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.
Severity Minor Likelihood Unlikely Evidence B

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

You're combining red clover with a birth control or hormone pill that contains ethinyl estradiol and norethindrone. The theory here is that red clover's isoflavones might slow down the CYP3A4 enzyme that helps clear these hormones, which could in theory let their levels creep up a bit. Higher estrogen levels can mean things like breast tenderness, nausea, bloating, or breakthrough spotting.

The good news: this is mostly a lab-dish concern. When researchers actually gave people standardized red clover capsules and tested a CYP3A4 marker drug, they saw no real change. So this interaction is rated minor and unlikely. Still, since red clover has its own estrogen-like activity, it's worth a quick chat with your pharmacist or doctor before adding it.

About Red Clover

What is Red Clover?

Red Clover
  • Menopause symptoms (hot flashes)
  • High cholesterol
  • Bone health/osteoporosis
  • Skin and hair concerns
  • Women's health

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.
