Why this applies to Ibuprofen

Ibuprofen falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: your body uses a liver enzyme called CYP2C9 to clear ibuprofen. In a test tube, red clover's plant estrogens look like they might slow that enzyme down a little. If that happened in real life, ibuprofen could hang around a bit longer, which in theory might mean more of its usual side effects like stomach upset, heartburn, or fluid retention.

The good news is this is mostly lab-dish thinking. When researchers actually gave people red clover isoflavones and tracked a similar CYP2C9 drug, they saw no real change. So for most folks taking ibuprofen now and then, this interaction is unlikely to matter. Still, if you take ibuprofen regularly or have stomach or kidney concerns, run it by your pharmacist or doctor first.

Ibuprofen also falls under ANTICOAGULANT/ANTIPLATELET DRUGS.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

Here's the gist with red clover and ibuprofen: both can have a mild effect on how easily your blood clots. Ibuprofen, like other NSAIDs, makes platelets a little less sticky, and lab studies hint that an isoflavone in red clover (genistein) might do something similar. In theory, stacking the two could slightly raise the odds of bruising or stomach irritation and bleeding.

The good news: this is mostly test-tube speculation. Actual human studies, including ones lasting a year or two, found red clover did not measurably change blood clotting. So for most people the real-world risk is low. Still, if you take ibuprofen often or have a bleeding history, mention the red clover to your pharmacist or doctor first.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. Because of its hormone-like effects, it's not for everyone—talk with your pharmacist or doctor before trying it, especially if you have a hormone-sensitive condition.

What Is Ibuprofen Used For?

Brand names: Motrin, Children's Motrin, Advil, Children's Advil, Fenbid Forte Gel, Ibugel Forte, Arthrofen, Ebufac, Rimafen, Calprofen, Fenpaed, Galprofen, Nurofen for Children, Orbifen for Children, Brufen, Brufen Retard, Fenbid, Tab-Profen, Ibuprofen, Calodor

Oral ibuprofen products (tablets, coated tablets, and liquid-filled capsules) are used to temporarily relieve minor aches and pains from headache, backache, toothache, muscular aches, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever. The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).

Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
