Aspirin, Caffeine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

The caffeine in Anacin is cleared by a liver enzyme called CYP1A2. In a test tube, red clover's plant estrogens can slow that enzyme down a little. If that happened in real life, caffeine could linger longer and you might feel a bit more jittery, have a faster heartbeat, or trouble sleeping. The aspirin part of this combo isn't really involved here.

Here's the reassuring part: this is mostly a lab-bench finding. When researchers actually gave people red clover and then measured caffeine, they saw no real change. So this is a minor, unlikely concern. If you notice extra caffeine-type buzz, just mention it to your pharmacist, but you don't need to be alarmed.

Here's the short version for your Anacin (aspirin plus caffeine): on paper, red clover's plant compounds could slow down a liver enzyme called CYP3A4, and the caffeine part of this combo gets handled in part by that pathway. If clearance slowed, you might in theory feel caffeine a bit more strongly, think jitteriness, a racing heart, or trouble sleeping.

But here's the reassuring part. When researchers actually tested red clover in people, it didn't meaningfully change how the body processed a CYP3A4 test drug. So this concern is mostly theoretical, and it's rated minor and unlikely. I wouldn't lose sleep over it. If you take red clover regularly, just mention it to your pharmacist so they can double-check the rest of your list.

Although some laboratory research suggests that red clover may have anticoagulant and antiplatelet activity, clinical research has not shown this effect.
In vitro research suggests that genistein in red clover has antiplatelet effects, and historically, red clover was thought to have anticoagulant effects due to its coumarin content. However, some experts state that this is unlikely as most natural coumarins have not been shown to have anticoagulant effects, and their content in red clover is low. Additionally, some clinical research in postmenopausal patients found no effect on coagulation or prothrombin time with the use of red clover flowering tops 378 mg daily for 12 months or red clover isoflavone (Rimostil) 50 mg daily for 2 years.

If you take a combo product like Anacin (aspirin plus caffeine), here's the honest picture with red clover. The thinking is that red clover might have a mild blood-thinning or antiplatelet effect, which could in theory add to what aspirin already does to slow blood clotting. On paper that raises the idea of slightly more bruising or bleeding, like nosebleeds or a cut that takes longer to stop.

But here's the reassuring part: this concern comes mostly from lab and test-tube studies. Actual studies in people, including postmenopausal women taking red clover for up to two years, found no real change in clotting. So this interaction is rated minor and unlikely. Still, if you take aspirin regularly, it's worth a quick word with your pharmacist before adding red clover.

Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

Here's the gist with Anacin, which pairs aspirin with a dose of caffeine. The interaction on file is really about the caffeine part. One of red clover's compounds, genistein, can in theory slow down how your body clears caffeine. If that happened, caffeine could linger a bit longer and you might feel jittery, have a racing heart, or trouble sleeping.

The reassuring part: this only showed up in a study using very high, isolated genistein (1 gram a day). At the much smaller isoflavone amounts found in actual red clover supplements, studies found no meaningful effect on caffeine. So for most people this is a very low concern. Still, if you take red clover regularly, it never hurts to mention it to your pharmacist.
