Aspirin, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Danshen interacts with.

Theoretically, danshen may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
Danshen has been reported to have antithrombotic effects. Animal research also suggests that taking a danshen combination formula with clopidogrel exhibits a synergistic increase in antiplatelet aggregation and prolongation of coagulation time when compared with either taken alone.

Here's the thing to keep an eye on with this combo. The aspirin in Anacin already works by making your platelets less sticky, so your blood clots a little slower. Danshen seems to do something similar, it appears to thin the blood and slow clotting too. Stack the two and those effects can add up, which may raise your chance of bleeding. That could show up as easy bruising, nosebleeds, bleeding gums, or stomach upset (aspirin can already irritate the stomach lining).

I'll be honest, this warning rests mostly on animal studies and scattered case reports, so it isn't rock solid. Still, bleeding is serious enough that I'd play it safe. Please check with your pharmacist or doctor before pairing Danshen with Anacin rather than changing anything on your own.

Theoretically, danshen may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that danshen tincture and various constituents of danshen inhibit the activity of CYP1A2. However, this activity has not been shown in humans when theophylline, a CYP1A2 substrate, was used as a target drug.

Here's the thing with Anacin: it pairs aspirin with caffeine, and it's the caffeine part that matters here. Caffeine is cleared from your body by a liver enzyme called CYP1A2. In lab and test-tube studies, Danshen seems to slow that enzyme down. If that happened in real life, caffeine could linger longer and you might feel jittery, anxious, have a racing heart, or trouble sleeping.

That said, I want to keep this in perspective. This is theoretical, based only on lab work. When researchers tested Danshen in actual people using a similar drug, they didn't see this effect. So it's worth a mention, not a worry. If you regularly use both, just check in with your pharmacist or doctor first.

Danshen might alter the levels and clinical effects of drugs metabolized by CYP3A4.
Preliminary clinical research in healthy males shows that the administration of danshen for 10-14 days induces intestinal CYP3A4 and increases the clearance of midazolam, a CYP3A4 substrate. The maximum concentration of midazolam was decreased by 31% to 67%, and drug levels were decreased by 27% to 80%. However, a single dose of danshen has the opposite effect, increasing maximum concentrations of midazolam by 87%.

Here's the honest picture with Anacin (aspirin plus caffeine) and Danshen. The interaction on file is about CYP3A4, an enzyme in your gut and liver that helps clear certain drugs. The research showed that taking Danshen for about 10 to 14 days speeds this enzyme up, while a single dose can briefly slow it down. The bigger issue for this combo, though, is that Danshen may thin the blood, and aspirin already does that too.

So my real concern isn't the caffeine, it's the doubled-up effect on clotting. Together they could raise your chance of bruising, nosebleeds, or stomach bleeding. The evidence here is early and limited, but please loop in your pharmacist or doctor before combining them, especially if you take aspirin daily.

Theoretically, danshen may increase the levels of aspirin and the risk of bleeding.
Research in healthy adult males shows that taking a combination of danshen and kudzu with aspirin increases plasma aspirin area under the curve by approximately 3.4-fold. Animal research also shows that taking a combination of danshen and kudzu (danshen-gegen formula) with aspirin increases maximal blood levels of aspirin and salicylic acid by approximately 4-fold and 3.7-fold, respectively, without impacting blood loss. Taking danshen increases the antiplatelet activity of aspirin and might increase the side effects of aspirin.

Anacin combines aspirin with caffeine, and it's the aspirin part that matters here. Danshen can do two things at once: studies in healthy men suggest it can raise the amount of aspirin in your blood (one combination boosted aspirin levels several-fold), and danshen on its own can thin the blood a bit too. Aspirin already nudges your platelets toward not clotting, so stacking these together pushes in the same direction.

What that can look like in real life is easier bruising, nosebleeds, bleeding gums, or stomach upset and irritation from the aspirin. This is a probable, moderate interaction, so please check with your pharmacist or doctor before combining them rather than changing anything on your own.
