Belladonna, Caffeine, Ergotamine, Pentobarbital falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
This combination product contains caffeine, which your body clears using a liver enzyme called CYP1A2. In lab (test tube) studies, red clover slowed that enzyme down a bit. If that happened in real life, caffeine could linger a little longer, which might mean more jitteriness, a racing heartbeat, or trouble sleeping.
Here's the reassuring part: when researchers actually gave people standardized red clover isoflavones twice a day for two weeks, it did not change how caffeine was handled. So this is a theoretical, minor concern based on weak evidence, not something that's been shown to cause problems. The belladonna, ergotamine, and pentobarbital pieces aren't really involved here. Still, if you notice anything unusual, check with your pharmacist or doctor.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

This combo product (often used for headaches) leans on the ergotamine piece, which your body clears partly through a liver enzyme called CYP3A4. In a test tube, red clover looked like it might slow that enzyme down, which in theory could let ergotamine build up. With ergotamine that's worth noting, because too much can cause cold or tingling hands and feet, leg cramps, nausea, or a pounding headache. The caffeine, belladonna, and pentobarbital pieces have their own effects too.
Here's the reassuring part: a real human study found red clover did not meaningfully change how a CYP3A4 medication was handled, so a true problem is unlikely. Still, before pairing them, it's smart to run it by your pharmacist or doctor.

CAFFEINE

Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

The interaction here is really about just one part of this combination pill: the caffeine. In very large amounts, the genistein in red clover could in theory slow how fast your body clears caffeine, which might make caffeine's effects (jitteriness, a racing heart, trouble sleeping) hang around a little longer. But this is largely a paper concern. Studies using actual red clover doses, standardized to 60 mg of isoflavones twice daily, found no change in how caffeine behaves.
So for a typical red clover supplement, I wouldn't expect a meaningful problem with Micomp PB. Since this product also contains ergotamine and a barbiturate, it's still wise to run any new supplement past your pharmacist or doctor first.
