Ephedra and Belladonna, Caffeine, Ergotamine, Pentobarbital

These two interact in 5 distinct ways. Each one is explained separately below.

Why Are There 5 Interactions?

Ephedra interacts with whole categories of medication. Belladonna, Caffeine, Ergotamine, Pentobarbital falls into 5 of those categories, so it can interact with Ephedra in 5 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Major).

Major

STIMULANT DRUGS

1 of 5

Why this applies to Belladonna, Caffeine, Ergotamine, Pentobarbital

Belladonna, Caffeine, Ergotamine, Pentobarbital falls under STIMULANT DRUGS — the group Ephedra interacts with.

Theoretically, concomitant use might increase the risk for serious adverse effects.
Drugs with CNS stimulant properties might increase the risk of hypertension and the adverse cardiovascular effects of ephedra.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination medicine (Micomp PB) is a mix of ingredients, and one of them, caffeine, is a stimulant. The product also contains ergotamine, which tightens blood vessels, plus belladonna and the sedative pentobarbital. The worry here is mostly about stacking stimulants. Ephedra is itself a powerful stimulant, so pairing it with the caffeine in this product can pile on the same effects: a racing or pounding heart, jitteriness, and higher blood pressure. Ephedra plus ergotamine's blood-vessel tightening could add even more strain on your circulation.

I'll be honest, this is based on anecdotal reports rather than strong studies, but the potential harm is serious enough that I wouldn't risk it. Ephedra is banned in U.S. supplements anyway. Please talk with your pharmacist or doctor before combining anything like this.

METHYLXANTHINES

2 of 5

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under METHYLXANTHINES.

Concomitant use might increase the risk of serious adverse effects.
Use of ephedra with caffeine or other methylxanthines such as theophylline might increase the risk of stimulatory adverse effects. There is also some evidence that using ephedra with caffeine might increase the risk of serious life-threatening or debilitating adverse effects such as hypertension, myocardial infarction (MI), stroke, seizures, and death.
Severity Major Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

The product you're asking about (Micomp PB) contains caffeine along with belladonna, ergotamine, and pentobarbital. Caffeine is a methylxanthine, a stimulant. Here's the problem: ephedra is also a powerful stimulant. Stacking two stimulants together can really pile onto your heart and nervous system. People have reported things like a racing or pounding heart, a sharp jump in blood pressure, jitteriness, headache, and trouble sleeping. In serious cases, the combination has been linked to dangerous events like heart attack, stroke, and seizures.

This is based mostly on anecdotal reports, not large trials, but the risk is taken seriously enough that I'd steer clear. Keep in mind ephedra is actually banned in U.S. supplements. Please talk with your pharmacist or doctor before combining anything like this, and ask about safer options.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

3 of 5

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES.

Theoretically, ephedra might decrease levels of drugs metabolized by CYP1A2.
Some animal research suggests that ephedra induces CYP1A2 and increases the clearance of CYP1A2 substrates such as caffeine.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product (Micomp PB) contains caffeine, and caffeine is the part this interaction touches. Animal studies hint that ephedra may speed up a liver enzyme called CYP1A2, which is the main enzyme that clears caffeine. In theory, that could make your body burn off the caffeine a bit faster, slightly weakening it. Keep in mind this is weak, lab-and-animal-level evidence, so I wouldn't lose sleep over it.

The bigger concern isn't the enzyme at all. Ephedra is a strong stimulant, and stacking it on top of the caffeine here can pile on jitteriness, a racing or pounding heart, higher blood pressure, and trouble sleeping. Ephedra is also banned in U.S. supplements for safety reasons. Please talk with your pharmacist or doctor before combining anything like this.

ANTICONVULSANTS

4 of 5

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under ANTICONVULSANTS.

Theoretically, ephedra may reduce the effects of anticonvulsants.
Ephedra has been associated with reports of seizure.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

This combination product (Micomp PB) contains pentobarbital, a barbiturate that calms the nervous system and can act like a seizure threshold raiser. Ephedra works in the opposite direction. Its stimulants, ephedrine and pseudoephedrine, rev up your fight or flight system and have actually been linked to seizures in some reports. So in theory, ephedra could push against the calming, anti-seizure side of this medicine and make it less reliable.

I'll be honest with you: this concern is based on how these ingredients work, not on strong studies, so it's theoretical. Still, ephedra is banned in U.S. supplements for good reason (strokes, dangerous blood pressure spikes), and stacking a stimulant against a sedative is asking for trouble. Please don't combine them without talking to your pharmacist or doctor first.

ERGOT DERIVATIVES

5 of 5

Belladonna, Caffeine, Ergotamine, Pentobarbital also falls under ERGOT DERIVATIVES.

Theoretically, concomitant use might increase the risk of hypertension.
The ephedrine contained in ephedra might cause excessive vasoconstriction and hypertension when used in combination with ergot derivatives.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your Micomp PB product contains ergotamine, which tightens blood vessels to help with migraines. The caffeine in it is a mild stimulant too. Here's the concern: ephedra contains ephedrine, another stimulant that narrows blood vessels and raises blood pressure. Stacking these two vessel-tightening effects together can push things too far.

Realistically, the worry is a sharp rise in blood pressure, a pounding or racing heart, cold or tingling hands and feet, and headache. Both ergotamine and ephedra can squeeze blood vessels on their own, so combining them adds risk. Honestly, ephedra is banned in U.S. supplements for good reason. Please talk with your pharmacist or doctor before using anything containing it alongside this medication.
