Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the gist for your Midol Max Strength. The caffeine in this combo is cleared by a liver enzyme called CYP1A2. In a test tube, red clover's plant estrogens (isoflavones) can slow that enzyme down, which in theory could let caffeine linger a bit longer and add to things like jitteriness, a racing heart, or trouble sleeping.

But here's the reassuring part: when researchers actually gave healthy people a standardized red clover dose and tracked caffeine, they saw no real change. So this is mostly a lab finding, rated minor and unlikely. The acetaminophen and pyrilamine pieces aren't a concern here. If you're sensitive to caffeine, just keep an eye on how you feel, and check with your pharmacist if you have questions.

Here's the gist if you're reaching for Midol Max Strength along with red clover. In a test tube, red clover's plant compounds looked like they might slow down CYP3A4, one of the liver enzymes that helps process certain medicines. The worry was that this could let drug levels creep up. But when researchers actually tested red clover in people, it didn't change how the body handled a CYP3A4 medication at all.

For this combo product, that's reassuring. None of these three ingredients (acetaminophen, caffeine, or pyrilamine) is a prodrug that needs activating, and they aren't heavily dependent on CYP3A4 anyway. So a real-world problem like extra drowsiness or jitteriness is unlikely. Still, since acetaminophen is hard on the liver, keep your dose sensible and check with your pharmacist if you take it often.

Theoretically, soy might reduce the clearance of caffeine; however, a small clinical study found no effect.
Red clover contains genistein. Taking genistein 1 gram daily for 14 days seems to inhibit caffeine clearance and metabolism in healthy females. However, this effect does not seem to occur with the lower amounts of genistein found in red clover. A clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine.

Midol Max Strength has three ingredients, but the one red clover could theoretically touch is the caffeine. The idea is that an isoflavone in red clover called genistein might slow how your body clears caffeine. If that happened, caffeine could hang around a bit longer, leaving you feeling jittery, restless, or making it harder to sleep.

Here's the reassuring part: this only showed up at very high genistein doses (about 1 gram daily). At the modest isoflavone amounts found in typical red clover products, studies in healthy people found no real change in caffeine levels. So for most folks this is a minor, mostly theoretical concern. Still, if you notice extra jitteriness, mention it to your pharmacist or doctor.
