Ephedrine Sulfate, Hydroxyzine, Theophylline falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.

Theoretically, red clover might increase levels of drugs metabolized by CYP1A2; however, a small clinical study found no effect.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

In this combination product, the ingredient that matters most here is theophylline, which your liver clears using an enzyme called CYP1A2. In a test tube, red clover's isoflavones can slow that enzyme down a bit. If that happened in your body, theophylline could build up, and since it has a narrow safety window, that might mean a racing heart, jitteriness, nausea, or trouble sleeping (effects ephedrine could add to).

The good news: this worry is mostly theoretical. A small human study using standardized red clover isoflavones found no real change in how the body handles a similar CYP1A2 substrate. So the risk is low. Still, because theophylline needs careful monitoring, please run this past your pharmacist or doctor before combining them.
