Here's the quick version: omeprazole is cleared by a liver enzyme called CYP2C19, and lab studies hint that red clover may weakly slow that same enzyme down. If that happened in real life, your body would clear omeprazole a little more slowly, so its levels could drift up slightly. That might mean a touch more acid-blocking effect, or mild side effects like headache, stomach upset, or loose stools.

I want to be honest about how thin this is, though. It comes only from test-tube research and has never actually been seen in people, so it's rated minor and unlikely. For most folks, taking red clover with omeprazole isn't a real worry. Still, it's always smart to run your supplement list by your pharmacist or doctor.

Theoretically, red clover might increase levels of drugs metabolized by CYP2C9; however, a small clinical study found no effect.
In vitro evidence suggests that red clover might inhibit CYP2C9. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of tolbutamide, a CYP2C9 probe substrate.

Here's the short version: red clover and omeprazole are very unlikely to cause a real problem together. The concern on paper is that red clover's isoflavones might slow down an enzyme called CYP2C9, which is one of the pathways your body uses to clear omeprazole. If that happened, omeprazole levels could creep up a bit, possibly nudging side effects like headache, nausea, or stomach upset.

But this idea comes only from lab (test tube) studies. When researchers actually tested red clover in people using a similar CYP2C9 drug, they saw no meaningful change. So the worry stays theoretical. Omeprazole is not a prodrug, so it still works normally here. If you take both, just mention it to your pharmacist to be safe.

Here's the short version: this one isn't worth losing sleep over. The concern is that red clover's plant estrogens (isoflavones) might slow down CYP3A4, one of the liver enzymes that helps clear omeprazole. If that happened, omeprazole could build up a little, possibly nudging up side effects like headache, stomach upset, or diarrhea. Omeprazole is a regular drug here, not a prodrug, so more in your system just means a slightly stronger effect.

But the real-world evidence points the other way. When researchers actually tested red clover capsules in people, they saw no meaningful change in a similar CYP3A4 drug. So this is mostly theoretical. Still, if you start red clover, give your pharmacist a quick heads-up just to be safe.

Red clover is a plant rich in isoflavones (plant compounds with weak estrogen-like activity) that is most often used for menopause symptoms like hot flashes. The evidence is mixed and generally weak, with some studies showing modest benefit and others showing none. About Omeprazole

What Is Omeprazole Used For?

Brand names: Prilosec, Zegerid, Losec

Omeprazole is used to treat a range of conditions where stomach acid causes problems. These include heartburn and acid reflux (GERD), erosive esophagitis (damage to the esophagus from acid), and stomach or duodenal ulcers. It can also be used alongside antibiotics to help get rid of an H. pylori infection — a bacteria that causes ulcers.

In more serious cases, omeprazole is used to manage conditions where the stomach produces far too much acid, such as pathologic hypersecretory conditions (rare medical disorders that cause extreme acid overproduction). It is approved for adults and children one year of age and older for GERD and erosive esophagitis.
