Acetaminophen, Phenylephrine, Chlorpheniramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Red Clover interacts with.
In vitro evidence shows that red clover inhibits CYP1A2. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of caffeine, a CYP1A2 probe substrate.

Here's the honest picture with this combo cold medicine and red clover. In a lab dish, red clover can slow down an enzyme called CYP1A2, which helps your body clear certain drugs. In theory, that could let levels of an affected medicine creep up a bit. But when researchers actually tested red clover isoflavones in real people, it did not meaningfully change how the body handled a CYP1A2 substrate.
For your Super Cold Tabs (acetaminophen, phenylephrine, and chlorpheniramine), none of these are prodrugs, and this enzyme isn't their main clearance route. So a real-world problem here is unlikely. Just keep an eye out for the usual chlorpheniramine drowsiness or phenylephrine jitteriness, and check with your pharmacist if you have questions.

In vitro evidence shows that red clover might inhibit CYP3A4 isoenzymes. However, a clinical study in healthy postmenopausal individuals shows that taking red clover capsules standardized to contain 60 mg isoflavones twice daily for 14 days does not affect the pharmacokinetics of alprazolam, a CYP3A4 probe substrate.

Here's the short version: this combination is very unlikely to cause you trouble. Red clover can weakly block a liver enzyme called CYP3A4 in test-tube studies, and a few ingredients in this cold tablet pass through that same pathway. In theory, slowing that enzyme could let a drug build up a little. But when researchers actually tested red clover in people, it did not change how the body handled a standard CYP3A4 medication.
So in real life, taking red clover with this Acetaminophen, Phenylephrine, and Chlorpheniramine product is not expected to noticeably boost side effects like drowsiness, dry mouth, or a jittery, raised blood pressure feeling. Still, multi-symptom cold products have their own cautions, so check with your pharmacist if you take it often or have heart or blood pressure concerns.
